How to apply for the NDIS
Service fees are pre-determined by the NDIA and published in an NDIS price guide.
Step 1
Check your eligibility
Visit the NDIS website, call 1800 800 110 or email enquiries@ndis.gov.au to find out if you’re eligible for the NDIS.
Step 2
Make an Access Request
You can make a Verbal Access Request (VAR) or fill out an Access Request Form (ARF). You can do this by phone, email, or at an NDIS or Local Area Coordinator (LAC) office.
Step 3
Get your access decision
The NDIA will write to you to confirm your eligibility for the NDIS. If you’re eligible, you’ll be contacted to arrange a planning meeting.
Step 4
Plan your support
Contact us and we’ll work with you to plan and provide your NDIS supports, so you can live the life you want.

Supporting you through the NDIS planning process
If you’d like support through the NDIS planning process, our team can help.
We’ll sit down with you and together, we’ll develop a plan that’s centred around the most important person in the process – you. It will include things like your likes and dislikes, your passions and goals and is designed to assist you in accessing the NDIS support you need to live the life you choose.
Preparing for a plan review
If Uniting is delivering support through your NDIS plan, please get in touch with us at least 8 weeks prior to your NDIS plan review date so we can help you prepare and support you to get the best outcome from your plan review.
This might include things like supporting you to identify and gather the evidence you need to successfully apply for more funding.
